持ち物

　自治体から届いた追加（3回目）接種に係る「封筒に入っている書類一式」を持参してください。

学生証又は職員証（左記を持たない者は身分を証明できるもの：運転免許証，在留資格証，健康保険証等）

※ 自治体から届いた追加（3回目）接種に係る封筒に，同封されています。持参していない場合は，接種できません。

接種券一体型予診票（追加接種用）（PDF）
※ 住民登録上の住所に送付された接種券一体型予診票に，記入例を参考に必要事項を記入の上，必ず持参してください。持参していない場合は，接種できません。

What to bring for the booster shots (the 3^rd dose)

　Please bring the set of documents related to the booster shots (the 3^rd dose) of COVID-19 sent from your municipality by mail.　

Student ID card/staff ID card
Identification documents: Residence Card, Driving License, Health Insurance Card etc. (if you don’t have the above)

Certificate of vaccination
※ It is enclosed in the envelope sent from your municipality. If you don’t bring it, you will not be vaccinated.

使用ワクチン Type of vaccine

本学職域追加接種で使用するワクチンは「武田／モデルナ」社製です。
※ 武田／モデルナ社の新型コロナワクチンについて | 厚生労働省Webサイト
「武田／モデルナ」社のワクチンにおける3回目接種は，1・2回目接種で用いた量の半量となります。
2回目接種後と比較して，発熱や疲労などの接種後の症状が少ないことが報告されています。
ワクチンの接種費用は「無料」です。
本ワクチンは接種された方の新型コロナウイルス感染症の発症を予防するワクチンです。
1回目や2回目の接種に用いたワクチンの種類に関わらず，本ワクチンの接種が可能です。
本ワクチンの追加接種を受けても，感染を完全に予防できるわけではありません。ワクチン接種にかかわらず，引き続き，適切な感染予防対策（マスク着用，密集・密接・密閉の回避，手洗いや咳エチケット等）を行ってください。
本学職域追加接種は，ワクチン接種希望者が自治体や医療機関等で接種することを妨げるものではありません。
基礎疾患，アレルギー等のある方は，かかりつけ医と事前に相談してください。
Takeda/Moderna vaccine.
※ Instructions for the COVID-19 vaccination for Takeda/Moderna vaccine
The amount of the booster shots (the 3^rd dose) of the Takeda/Moderna vaccine is 0.25ml, which is half amount of the first and second doses.
It has been reported that there are fewer symptoms such as fever and fatigue compared to after the second dose vaccination.
The vaccination is free of charge.
This vaccine prevents the onset of COVID-19.
You can receive this vaccine regardless of which type of vaccine you received for your first or second dose.
Receiving a booster dose of this vaccine will not completely prevent infection. You should continue to take appropriate infection prevention measures (wearing a mask, avoiding the “three Cs” (crowded places, close contact settings, closed spaces), washing hands, cough etiquette, etc.).
Implementation of the booster shots (the 3rd dose) at KU does not mean that you cannot get the 3^rd dose at your municipal or other medical institutions.
If you have any underlying medical conditions or allergies, please consult your primary care doctor or other healthcare professional prior to the vaccination.
